Social Prescribing Link Workers

Many things can affect your health and wellbeing. Maybe your feeling isolated or lonely. Perhaps you have money worries or feeling stressed through work. 

This is where Social Prescribing comes in.

But how do you make contact with a Social Prescribing Link Worker?

Well it starts with a conversation. This could be with your Doctor or another member of the Practice Team. They can then refer you to a Social Prescribing Link Worker.

Your Link Worker will spend time listening to you and what your worries or concerns maybe.

The Link Worker will help put you in touch with people or activities that might help you feel better.

They could introduce you to a new community group or a local club.

Maybe they will help you find legal advice or help with debt.

It could just be a matter of finding you some information  on local resources.  Or help with completing a form.

Whatever it may be, Social Prescribing can help you have to have more control over your own health and find ways to improve how you feel in a way that suits you.

If this is something you think would benefit you, speak to your GP or a member of the Practice Team and they can refer you into one of our Link Workers.
