Doctor's Desk

Clinical Pharmacists

Our Clinical Pharmacists are working closely with your GP Surgeries to support you with all your medication-related queries, as well as chronic disease management. They use their knowledge and expertise to help you make the best use of your medicines, keeping your ideas, concerns and expectations at the centre of every consultation, resulting in a holistic review.

Improving Mental Health

To improve access to mental health care we have introduced our Healthy Minds Hub. Through this we have a number of different routes we can explore, such as working with local organisations to provide Psychological Therapy trainees to our patients. We are also exploring an online therapy service which may benefit very many of our patients.

Improving Weight Management

Our Health & Wellbeing Team are working closely with patients who have an interest in taking control of their weight management. Current services do not always offer the personalised care that is needed in some cases. This is where our services offer a more personalised level of care aiming to help patients to take control and improve quality of life.

Enhanced Access

As part of our commitment to improving access to General Practice, we are working with our GP surgeries to offer various appointments during the evenings and weekends.

Improving Cancer Care

Anyone registered with our practices, upon diagnosis of a cancer, should be offered the opportunity to speak to one of our Social Prescribing Link Workers. This can be really helpful in gaining extra support at a difficult time, as well as help and expertise about and social, emotional or financial support that you maybe entitled to.

SilverCloud - Mental Wellness

We have partnered with SilverCloud, an online provider of a Mental Wellness platform that gives you the tools to improve your health and wellbeing. In order to access the service please visit the SilverCloud Signup Page and use the PIN code WEPCN if prompted.

Social Prescribing

Social Prescribing links patients to services. These services can help with financial services, care packages, befriending and even community groups. They have particular expertise in helping those over the age of 75 access support and services that maybe of benefit. As such we have been proactively contacting patients who feel may benefit from our help. By linking people with services we hope to improve the Health & Wellbeing of our community.

This is where our services offer a more personalised level of care int eh aim to reduce pain and improve quality of life.

Working with Learning Disabilities

As well as offering support to their designated GP surgeries, our surgery Care Co-Ordinators are taking on the role of Learning Disability Champions. This role will be supporting surgeries to gain new official recognition as a 'Learning Disability Friendly Practice'. This is the next step in helping to reduce health inequalities in our area and make our people and communities fairer, stronger and more connected.
