CLINICAL PHARMACY
Clinical Pharmacisits
All our clinical pharmacist team work in house in our surgeries and focus on the management associated with long term medical conditions. You may also encounter the clinical pharmacist team as part of your annual medication review, at which point they will review your medication alongside your annual blood tests and monitoring.
Some of our clinical pharmacists are also independent prescribers and will work with you to optimise the management of your conditions.
In addition, the PCN network also supervises the training of clinical pharmacists completing their general practice clinical pharmacy training, included becoming independent prescribers. Furthermore, some of our clinical pharmacists have a keen interest and speciality in certain clinical areas, such as diabetes and frailty and have focussed clinics around these areas.
Pharmacy Technicians
Our pharmacy technician, has a varied role on our PCN network, she has assisted with multiple successful projects in the past such as identifying patients with elevated bloods pressure readings and referring them to the correct clinical staff member for management. Currently they are piloting a post discharge medication review service, with the aim to improve the transfer of care between secondary and primary care.
Structured Medication Reviews
The clinical pharmacists are here to support you with all your medication-related queries. You can book an appointment with them for a medication review, which is often referred to as a ‘Structured Medication Review’. This thorough review allows the clinical pharmacist to review your medicines in line with national/local ​guidance, recent blood tests and other investigations.
The medication reviews can also overlap with long term conditions and therefore the clinical pharmacists also support the East Warwickshire PCN practices in chronic disease management. They use their knowledge and expertise to help you make the best use of your medicines, keeping your ideas, concerns and expectations at the centre of every consultation, resulting in a holistic review.
