Care Coordinators

Care Coordinators play an important role within our PCN providing the coordination and navigation of care and support across health and care services.

Working as part of our PCN's multidisciplinary team, coordinating referrals into Social Prescribing Link Workers, Health and Wellbeing Coaches, along with the groups sessions they run - Weight Management, Pain Management and Menopause Groups.

Within our PCN, the Care Coordinators help patients through our Healthy Minds Hub. Having the time to take a person centred approach and allowing the patient to make their own choices, which in turn allows the patient to feel empowered knowing they are being listened to.
